Job Summary:

The people we serve live in Hammer’s group homes or apartment programs with the support of care teams who provide on-going assistance, teach independent living skills, support health and personal cares, help with household tasks, and offer social and emotional support.

- Training: Completes and passes required training on-time.

Supervisor: Program Manager
Job Summary:
The Lead Direct Support Professional position directly supports individuals with intellectual and developmental disabilities. The people we serve live in Hammer’s group homes or apartment programs with the support of care teams who provide on-going assistance, teach independent living skills, support health and personal cares, help with household tasks, and offer social and emotional support.
In addition to providing on-site care for Hammer Residences individuals, this role will provide leadership and guidance to our Direct Support Professionals (DSP) and Careerist positions, in addition to administrative tasks, in collaboration with the Program Manager.
This position could be promotional opportunity for our Direct Support Professional (DSP) and/or Careerist positions.
Essential Functions:
- Communication: Reads email and communication book every scheduled shift. Demonstrates effective communication with co-workers and families.
- Training: Completes and passes required training on-time.
- Schedule: Is on-time for every scheduled shift, meeting attendance policy standards.

- Activities: signing individuals up for activities that they are interested in, keeping the program calendar up to date, communicating with team re: plans, etc.
- Shopping: purchasing groceries and supplies for the program.

Supervisor: Program Director
Job Summary:
- The Program Manager is responsible for overseeing the medical, financial, social and emotional needs of individuals served as well as day-to-day operations of the residential program for adults with intellectual and developmental disabilities (may include individuals with complex medical and behavioral support needs).
- The Program Manager supervises and leads direct care staff and ensures that services and activities comply with licensing regulations and with Hammer’s policies and person-centered philosophy, exercising good judgement to adapt and apply the guidelines to specific situations.
Essential Functions:
- Plan, implement and evaluate the programs, services and activities provided in the program. Facilitate the interdisciplinary team in developing plans to meet the individuals’ needs.
- Use required technology to maintain daily, monthly and annual required documentation. Completes all documentation on time.
- Ensure the program’s operations comply with DHS regulations as well as health and safety standards.
- Provide supervisory oversight, technical guidance and advice to staff.
- Responsible for scheduling staff and ensuring adequate staffing levels at your house. Working with support teams to work towards better staffing levels.
- Continually provide feedback to staff about performance; take appropriate action as warranted by performance. Complete yearly performance reviews.
- Ensure that current and new personnel complete training; provide orientation according to Hammer practices.
- Conduct staff meeting; review plans and reports to ensure quality.
- Respond to emergencies and report accidents and injuries.
- Maintain facility cleanliness, safety and compliance.
- Maintain operating budget, authorize expenses, and request necessary maintenance and repairs for the residence and vehicle(s).
- Prepare the program for licensing inspections.
- Completes Annual meetings on time regularly. Also includes Lead DSP staff as able.
- Demonstrate teamwork, cooperation and effective working relationships with individuals served, families, coworkers, supervisors, program team, professionals and others to facilitate quality services and a positive organization reputation. Respond to any complaints or incident reports promptly.
- Assist with all areas of the individuals’ programs including personal cares, medication administration, daily household chores and support in the community.
Job Specifications:
- Requires relevant education and experience to work independently.
- Four-year college degree in the social services or related field is preferred
- Relevant experience may be substituted.
- Supervisory experience preferred. Demonstrate rational, objective decision-making.
- Excellent written and verbal communication skills including fluency in English and proficiency in Microsoft Office.
- Frequently organizes multiple responsibilities at once, requiring time management, organizational and reporting skills, and proven ability to achieve assigned deadlines.

- Available to work a flexible full-time schedule including some evenings and some weekends.
- Requires a valid driver’s license, personal auto and driving record that meets Hammer’s insurance requirements. Must receive a MN DHS Background study clearance.
- Requires frequent participation in repetitive activities including working several hours at a time with a keyboard and monitor and laptop computer.
- May require lifting up to 40 pounds (such as lifting a wheelchair into a van), pushing or pulling an individual in a wheelchair or while assisting with cares. Household chores and physical cares may include twisting/turning, kneeling/squatting and assisting with pivot transfers.
- Experience operating specialized medical or adaptive equipment, including lifting equipment preferred.
- Must be prepared to respond to hazardous situations; including infectious disease or aggression.
Supervisor: Director of Program Services
Job Summary:
The Program Director is supervisor and resource person for Program Managers. As a resource person, the Program Director will assist in areas such as staff development, personnel issues and programming issues.
The Program Director must be knowledgeable of the field of Developmental Disabilities, and be able to utilize new developments in the field.
The Program Director would be a liaison with the community for the purpose of public relations.
The Program Director must work effectively with the Management Team, support staff, consultants, and other service providers.
The Program Director will insure that program services are developed for the people being served in the homes.
The Program Director must follow all pertinent policies and procedures relating to the delivery of services and the general management of the organization.
Job Specifications:
Staff Supervision
Supervises Program Managers in all aspects of their job performance by coaching them regarding their performance, conducting formal performance reviews, monitoring the completion of their responsibilities, and assisting them in resolving programming and staffing issues. Program Director will maintain regular contact with Program Manager.
Assists the Director of Support and Recruiting with open positions. Terminates the employment of employees as appropriate with the Program Manager.
Reviews and approves Program Manager time cards and assists the Business Office staff in processing these. May complete or review completed time sheets from individual homes in the absence of Program Manager.
Approves permanent household schedule changes, and oversees, in a general way, the weekly scheduling of Direct Support Professionals, including support scheduling issues.
Intervention and Programming Services
On an on-going basis, monitors the delivery of services to insure they are being provided as planned.
Assists staff team in handling of neighborhood issues.
Pursues resolution to grievances of those being served.
Through appropriate means seeks to protect civil and legal rights of those being served.
Assists the Director of Program Services and Program Managers in evaluating individuals seeking placement in our facility and determining his/her appropriateness.
Assists the Director of Program Services and Program Manager in planning for discharge of individuals and in locating appropriate future services.
Attends intake and discharge meetings.
Oversees the use of Rule 40 programming and psychotropic medications, making sure all applicable rules and required procedures are followed.
Assists Program Manager in handling crises, including follow-up with outside agencies. In the absence of the Program Manager handles these crises and develops a plan of action to resolve them.
Meets with household staff occasionally to plan services and general household operation.
Makes household visits.
On a rotating basis carries an emergency cell phone and responds appropriately to calls received.
Assists the Program Manager in providing information to licensing surveyors and similar individuals, i.e. UR, QA, Health Dept. surveys.
Family Issues
Responds to these individuals’ needs as is appropriate, to maintain a program of services that meets the needs of those being served.
Staff Training
Completes orientation for new Program Managers following the new orientation checklist. May provide training for staff through presentation at All Staff and general orientation.
Program Supervision
Attends Management Team meetings and works with this team to plan general program services and facility operation.
Supports Program Managers at special meetings pertaining to individuals being served.
Planning and Budget
Meets with Program Manager and the Chief Financial Officer to develop the Program and medical portion of budget. Discusses the implementation of this budget for the homes with the Program Manager.
Policy and Procedures
Assists Program Administrator in the development of policies and procedures pertaining directly to the provision of services to the people in our programs.
Other Duties
Completes other duties as assigned by the Director of Program Services or CEO.
Assists with Special Projects, Task Forces, Hammer Happenings, and also first call for help during the day.
Qualifications
Bachelor degree in a social service-related field from an Accredited college or university. Social service-related fields would include, but not be limited to, Education, Special Education, Psychology, Social Work, and Sociology.
At least two years of experience in the field of Developmental Disabilities.
Ability to converse effectively with other professionals in the Developmental Disabilities field.
Genuine sensitivity to and interest in people with Developmental Disabilities.
Excellent written and verbal communication skills.
Ability to think critically and problem solve.
Ability to interpret and manage a budget.
Ability to work effectively as a team member and to manage a team of staff members.

Fulfill the role of HIPAA Privacy Compliance Officer: oversee all activities related to development, implementation and maintenance of Hammer’s privacy policies in accordance with applicable federal and state laws

Qualifications:
Meet the requirements for a Qualified Developmental Disability Professional (QDDP) designation: possess one year of full time experience working with persons with intellectual/developmental disabilities/related conditions and have a bachelor’s degree in a professional human services related field or be a registered nurse.
Five years supervisory experience: (Leadership Development or Organizational Development experience preferred)
Demonstrated success in maintaining strong partnerships and collaboration across teams and organizations
Comprehensive knowledge of federal and state licensing regulations and funding mechanisms that relate to Hammer Residences’ services.
Strong strategic thinking, critical thinking, decision-making and problem-solving skills.
Exceptional communication and presentation skills. Strong consulting and facilitation skills.
Proficiency in use of MS (Excel, Word, PowerPoint).
Personal background that does not include disqualifying criminal activity as outlined in DHS Rule 11 and Adult Foster Care licensing requirements.
Physical Requirements:
Work is generally performed in an office environment and is sedentary in nature but may require standing, walking, and lifting up to 10lbs.
