We have adapted to change and changed with the times, always focused on meeting the needs of the people we serve. With support from staff, volunteers, donors, and families, the people who live in our homes and apartments are given the opportunity to live their lives to the fullest. Homes & Apartments

Our homes and apartments are adapted as needed to meet the range of age and abilities with 24-hour staffing, state-of-the-art keyless entries, safety, security, private rooms, ample living space and on-staff nursing staff. Support Services

We continue to be a successful leader in the disability services industry. Our person-centered approach highlights each individual’s needs and what is important to them. We work daily to give voice and meaning to each person’s daily life.

Case Management & Care Coordination

We provide both Case Management and Care Coordination services. We contract with Hennepin County to provide Case Management services for many programs, and we work with Medica on Care

Hammer Travel

Hammer Travel offers a variety of travel opportunities for individuals with developmental disabilities. Travel provides unique and enjoyable experiences for everyone, and it is our goal to meet the needs of each of our travelers so they have a positive and fulfilling experience.

Homes & Apartments

We have 59 group homes in the west and east metro area, along with 9 apartment programs.

We employ over 550 dedicated staff members who support approximately 380 people in our group homes and apartments.

Our homes and apartments are adapted to meet the range of age and abilities with 24-hour staffing, state-of-the-art keyless entries, safety, security, private rooms, ample living space and on-staff nursing. When you place your loved one in our care, we ensure a positive, safe environment with well-educated, experienced, and certified professionals.

About Our Homes

We provide an array of support services daily in our homes and apartments. Our goal is to continually innovate to be able to provide person-centered care to the people we support.

Living life to its fullest means something different to each person we serve. Many people enjoy spending time with friends and having the support and ability to pursue their varying interests. In our homes and apartments, there are opportunities to participate in a variety of activities like going out to dinner, to plays, shopping, and attending both local community events and Community Life department events.

About Our Apartments

In the west metro area, we have 9 apartment programs throughout Hennepin County, that offer supported, but more independent living, adapting as needed to meet a wide range of ages and abilities. Most people living in our apartments work during the day and return home to spend time with their family, friends, and staff.

Each apartment building accommodates up to 14 people, many of whom do their own cooking and cleaning with the support of our 24-hour staff team. Staff members help with social schedules, appointments, medications, and daily activities, including grocery shopping and other shared duties.

Our apartment buildings have a community apartment with a living room, kitchen, and dining area for cooking, eating, playing games, and watching TV. It also serves as an office and sleeping room for our employees. People supported living in the apartments have access to other amenities within the complex, such as the community room, a pool, and an exercise room.

Support Services

We are one of the largest nonprofit disability service providers in Minnesota. Our person-centered approach highlights each person’s abilities and what is important to them. We work daily to give voice and meaning to each person’s daily life. Respecting all abilities is at the core of our mission, by providing the support needed for people with disabilities to live meaningful, self-directed lives.

Onsite Nursing Staff

Our on-site team of registered and licensed practical nurses are dedicated to maintaining the best possible health care and safety standards for the people we support. Our nurses administer solutions and provide support to our staff, ensuring that health and safety standards are upheld. Our nurses stay current with research and best practices by regularly attending seminars and conferences. They are also members of the Developmental Disabilities Nurses Association, a professional organization for nurses serving individuals with intellectual and other disabilities.

Health Care Support Services Team

Our health services team supports the people we serve on an individual level. We have a Health Care team consisting of five Nurses, a Director of Health Services, a nutritionist, and two Nurse Coordinators. This care team provides Health Care Services with access to a nurse on call 24/7.
Health care plans created for each person are annually reviewed and updated:

  • Medication administration monitored electronically
  • Regular assessment of health conditions
  • Recommendations for occupational, physical, and speech therapies as needed
  • Consultation with other healthcare professionals
  • Consultation with family members, guardians, social workers, and outside agencies

Our level of care is unsurpassed in the field of direct care, contributing significantly to our reputation as an industry leader.

Case Management & Care Coordination

We provide both Case Management and Care Coordination services, contracting with Hennepin County to provide Case Management services for many programs. We also work with Medica on Care Coordination services.

Hennepin Contracted Case Management

We have been providing Case Management services since 2007, and contract with Hennepin County to provide Case Management services for the following programs:

  • CADI Waiver – Community Access for Disability Inclusion
  • BI Waiver – Brain Injury
  • DD Waiver – Developmental Disabilities
    • Including those utilizing CDCS – Consumer Directed Community Supports
  • NonWaiver DD
    • Rule 185 County-funded Case Management
    • VA/DD – Vulnerable Adult/Developmental Disability Targeted Case Management

Our Case Managers work with clients of all ages and with a wide range of support needs. Our clients may live throughout Minnesota as long as Hennepin is the County of Financial Responsibility for their funding. The clients we support may live independently, with their family, or in residential settings and we work to support them in accessing the available services that best support their needs.

Our Case Managers provide support in various ways, including the following:

  • Assist with accessing County assessments
  • Annual service planning and ongoing monitoring
  • Referrals to necessary support services
  • Service coordination and advocacy with providers
  • Provide resources to community supports

Medica Care Coordination

We have been providing Care Coordination services since 2012, and contract with Medica to support members on Medica’s SNBC (Special Needs Basic Care) and SNBC Enhanced Plans, which are Managed Care plans for individuals enrolled in Medicaid.

Our Care Coordinators work with members ages 18 – 64 years old with various disabilities. Our members live throughout Minnesota within Medica’s designated service area, which includes 14 counties as of 2020. The members we support may live independently, receive outside support services, or live in residential settings.
